  • EMPLOYMENT HISTORY

    All applicants are required to give an accurate employment history on this application. All driver applicants to drive in interstate commerce must provide the following information on all prior employers during the preceding 3 years. List complete mailing address; street number, city, state & zip code. Applicants to drive a commercial motor vehicle (Includes vehicles having a GVWR of 26,001 lbs. or more, vehicles designed to transport 15 or more passengers in intrastate or interstate commerce) shall provide an additional 7 years information on those employers for whom the applicant operated such vehicle. (NOTE: List employers in reverse order starting with the most recent. Add additional sheet if necessary.)

    I understand the School Bus Driver or School Bus Attendant position for which I am applying is a position with a schedule that conforms to a "school year".

  • APPLICANT STATEMENT TO BE READ AND INITIALED BY APPLICANT

    I certify that answers given herein are true and complete and I authorize investigation of all statements contained in this application for employment as may be necessary in arriving at an employment decision. I hereby understand and acknowledge that, unless otherwise defined by applicable law, any employment relationship between the applicant and Trinity Transportation is an "at will" nature, which means that the Employee may resign at any time and the Employer may discharge Employee at any time with or without cause. It is further understood that this "at will" employment relationship may not be changed by any written document or by conduct unless such change is specifically acknowledged in writing by an authorized executive of this organization. I authorize Trinity Transportation/Trinity, Inc. to make such investigations and inquiries of my personal, employment, financial or medical history and other related matters as may be necessary in arriving at an employment decision. (Generally, inquires regarding medical history will be made only if and after a conditional offer of employment has been extended.) I hereby release employers, schools, health care providers and other person from all liability in responding to inquiries and releasing information in connection with my application. In the event of employment, I understand that false or misleading information given in my application or interviews, may result in discharge. I understand, also, that I am required to abide by all rules and regulations of the Employer. I understand that information I provide regarding current and/or previous employers may be used, and those employers will be contacted, for the purpose of investigating my safety performance history as required by 49CFR 391.23(d) and (e). I understand that I have the right to review information provided by previous employers; have errors in the information corrected by previous employers and for those previous employers to resend the corrected information to Trinity Transportation; and have a rebuttal statement attached to the alleged erroneous information, if the previous employer(s) and I cannot agree on the accuracy of the information.
